  • Adult Guardianship Forms Clinic – for uncontested adult guardianships.
    • Mondays at 10:30AM or Wednesdays at 1:00PM at the Milwaukee Co. Courthouse. The Examining Physician’s or Psychologist’s Report (Form GN-3130) must be completed before making an appointment. Book an appointment here or at the Milwaukee Justice Center front desk in Room G-9 of the Milwaukee County Courthouse.
